What does a well-lived life look like to you? What does a well-supported death look like to you?
Please consider joining Life Transitions Doula- Colleen Corrigan, Retired Family Physician- Jay Federman, and Family Nurse Practitioner-Jennifer Phelabaum in an Advance Care Planning Informational.
Together we will demystify the process, directives, and additional considerations that often bog down the advance care planning experience.
What you can expect
Understanding the directives, when they may be applicable, resource support (living will, health care proxy, MOLST, DNR, etc)
Discussion regarding life sustaining treatments- the true facts and considerations (feeding tube, breathing tube, CPR)
Overview of end of life and after death care rights and options
Structured opportunities for values-based reflection
The program begins with a stand-alone Advance Care Planning informational session, open to residents and community members, followed by an optional multi-session series for individuals who wish to engage more deeply in the planning process with Doula, Colleen Corrigan. July 22nd: Community Centered Care
Let’s explore how we want to navigate life’s greatest transitions and remember our inherent capacity to care for one another in a way that feels dignified and supportive. Consider joining the Vigil and Bodycare Workshops (outlined below) this Summer for hands on experience and continued conversations.
August 5th: Living Legacy (Meet outside the Library)
Living from this moment, we always have the opportunity to realign with our values and goals and have our legacy come to life. Does legacy matter to you? What is your legacy and how is your life currently reflective of this? Come and share your thoughts and experiences. Consider joining the Living Legacy workshop in the fall (more information coming soon).

“Remember, what we are afraid of isn’t really death but our ideas/beliefs about death.”
Let’s explore seeing in diverse ways by sharing about our own beliefs and practices and pulling from the greater web of culture, beliefs, and traditions.

Within this value-based approach, we discover, discuss, document and deepen into what a well-lived life and a well-supported death look and feel like, and we connect to the well of wisdom that will continue to ripple onward.
At its core, advance care planning is not about planning for death. It’s about:
Reorienting our relationship to mortality so that it can be naturally life-giving
Using that awareness to clarify how they want to live now
Translating that clarity into:
conversations
relational alignment
practical documentation
And then keeping it alive, iterative, and communal
So instead of looking at this type of planning as a task, we can connect to this as a practice that remains alive.
This is what makes these sessions and this curriculum different than other forms of advance care planning. It becomes applicable to this moment and the ways we continue to shape and support our lives.
